Researchers evaluated alternative strategies to mitigate age-related inflation of false results. In contrast to fixed models, an age-dependent strategy maintains a near-constant FPR of 5.4% across all age groups. Therefore, this approach offers a more balanced diagnostic performance. Moreover, it achieved a detection rate of 81.3% for trisomy 21. While a global 5% FPR threshold significantly reduces false positives, it also lowers the detection rate too much. Consequently, age-specific calibration provides the highest diagnostic efficiency. Additionally, practitioners must remember that binary classifications based on single risk estimates have inherent limitations. Ultimately, transitioning to individualized thresholds could refine screening accuracy for diverse patient populations.
