Teleneurology ward rounds provide a revolutionary solution for hospitals lacking specialized stroke expertise. A groundbreaking multicenter study recently compared remote video consultations with traditional on-site ward rounds during the subacute phase of stroke care. The findings, published in April 2026, suggest that teleneurological expertise not only matches but significantly exceeds on-site care in adhering to clinical guidelines. This high level of adherence is vital for improving patient outcomes and reducing hospital complications.

Optimizing Care through Teleneurology Ward Rounds

Maintaining high-quality care during the subacute phase of stroke is essential for patient recovery. However, many hospitals struggle to provide consistent on-site neurological expertise. This prospective study evaluated 501 patients across 15 primary care hospitals. Blinded external experts reviewed documentation for six critical quality domains, including etiological classification and secondary prevention. Interestingly, the results favored remote experts over local assessments.

Researchers found that teleneurology ward rounds achieved complete adherence to all quality criteria in 92% of cases. In contrast, on-site ward rounds met these standards in only 54% of patients. This absolute difference of 38 percentage points highlights a substantial gap in care quality that telemedicine can bridge effectively. Specifically, remote consultations showed superior performance in areas like risk assessment and aftercare recommendations. Furthermore, the most pronounced benefit appeared in secondary prevention strategies, where teleneurology was 21 percentage points more accurate.

Clinical Implications for Modern Stroke Systems

These results demonstrate that telestroke networks are not just for emergency thrombolysis in the emergency department. By integrating teleneurology ward rounds into routine inpatient care, healthcare systems can ensure every patient receives guideline-based treatment. Consequently, rural and primary care hospitals can provide a level of care once reserved for specialized stroke centers. This approach is particularly relevant for improving long-term recovery and reducing the risk of recurrent stroke. Moreover, the study supports a broader adoption of telemedicine to standardize clinical excellence across diverse geographic regions.

Why are teleneurology ward rounds effective for stroke care?

Teleneurology ward rounds ensure that specialized neurological expertise is available regardless of the hospital's location. This remote access facilitates higher adherence to evidence-based guidelines compared to on-site consultations by non-specialists.

What quality domains were improved by teleneurology?

The study showed significant improvements across six domains: etiological classification, neurological examination, risk assessment, diagnostic recommendations, secondary prevention, and aftercare. The most significant improvement occurred in secondary prevention protocols.
