A recent study published in the Journal of Neurosurgery explores the long-term stability of cortical language mapping in adult patients suffering from medically refractory epilepsy. Electrical stimulation mapping (ESM) serves as a critical tool for identifying functional regions before epilepsy surgery. Specifically, surgeons use this technique to locate sites where stimulation interferes with naming. However, the temporal stability of these sites in adults has remained unclear until now. Consequently, researchers conducted a long-term study to determine if these language sites shift over time.
The research team analyzed twenty-two patients who underwent repeated mappings separated by more than one year. On average, over eight years elapsed between the two surgical procedures. Using a Bayesian hierarchical model, the researchers assessed the anatomical stability of the naming sites. The results provide significant insights into the permanence of eloquent cortical regions.
The findings reveal substantial stability in most patients. In fact, the median distance between mapped sites across two interventions was only 0.6 cm. Furthermore, eighty-six percent of the naming sites identified during the second operation were within 1.5 cm of the initial locations. This suggests that the adult brain maintains a relatively fixed language architecture over long periods.
While the majority of patients showed stability, the researchers observed rare instances of relocation. For instance, two patients exhibited naming sites that shifted entirely beyond 1.5 cm. Therefore, while surgeons can generally rely on previous maps, they must remain cautious. This long-term series underscores that while the adult cortex is remarkably stable, neuroplasticity still exists in a small subset of the population.
