Esophageal adenocarcinoma (EAC) represents an aggressive malignancy with a rising global incidence. Recent preclinical research focuses on S1PR2 PET imaging using a novel radiotracer to better understand tumor progression. Sphingosine-1-phosphate (S1P) acts as a bioactive lipid mediator that regulates various cellular processes via five specific receptor subtypes. Among these, Sphingosine-1-Phosphate Receptor 2 (S1PR2) significantly influences cancer cell proliferation and metastatic potential. Consequently, targeting S1PR2 provides a promising avenue for improving diagnostic precision and therapeutic monitoring in patients with EAC.
Researchers evaluated the effectiveness of [18F]TZ9555, a potent and selective S1PR2 radiotracer, in mouse models of EAC. The study utilized the OE33 tumor model to measure tracer uptake in primary tumors and metastatic lymph nodes. Notably, PET imaging data revealed substantial uptake in these tissues compared to normal muscle. Specifically, the metastatic lymph nodes showed a 7.0-fold higher uptake ratio than muscle tissue. These results indicate that [18F]TZ9555 effectively visualizes S1PR2-positive lesions in vivo. Furthermore, autoradiography and immunohistochemistry confirmed these findings by showing high S1PR2 expression within the tumor microenvironment.
Single-cell RNA sequencing further elucidated the expression patterns of S1PR2 within these tumors. Interestingly, human EAC samples showed significantly increased S1PR2 expression in fibroblasts. This suggests that S1PR2 contributes to the supportive framework that drives cancer invasion. Additionally, treating cells with the inhibitor JTE013 reduced tumor cell proliferation. This treatment also decreased the expression of S1PR2 and α-SMA in cancer cells. Thus, blocking the S1PR2 pathway might limit the aggressive growth of esophageal cancer. These developments suggest that [18F]TZ9555 has significant clinical potential for assessing disease progression and predicting outcomes.
S1PR2 plays a critical role in cancer cell proliferation and invasion. It facilitates the growth of EAC cells through pathways often activated by conjugated bile acids. High S1PR2 levels correlate with aggressive disease behavior and metastatic spread to lymph nodes.
The [18F]TZ9555 tracer allows for selective visualization of S1PR2 expression using PET/CT scanners. It provides much higher uptake in metastatic lymph nodes and primary tumors than in skeletal muscle. Consequently, it offers a non-invasive method to predict cancer progression and monitor the efficacy of targeted therapies.
