Covert hepatic encephalopathy (CHE) presents a significant clinical challenge in hepatology, as it often remains undiagnosed despite its profound impact on a patient’s quality of life. Recently, a multicentre randomized controlled trial published in Aliment Pharmacol Ther has provided robust evidence that Rifaximin for Covert HE effectively improves cognitive performance. This finding is particularly relevant for clinicians managing cirrhosis-related complications, as early intervention can potentially prevent the progression to overt hepatic encephalopathy.
The study utilized an open-label randomized design involving 50 patients with cirrhosis and associated CHE. Researchers randomized participants to receive either rifaximin or no treatment for a period of 12 weeks. Beyond cognitive scores, the trial investigated several secondary clinical parameters. Rifaximin treatment led to a significant reduction in cirrhosis-related adverse events. While serum ammonia levels and overall gut microbial diversity did not show drastic changes between the groups, rifaximin specifically altered certain bacterial taxa. Specifically, there was a notable loss of the [Eubacterium] brachy group in patients receiving the drug. These selective changes in the gut environment may explain the systemic improvements observed without requiring a complete overhaul of the microbiome.
This study highlights the Stroop test as a highly sensitive tool for evaluating treatment response in patients with CHE. In contrast to other psychometric tests that might not capture subtle improvements, the Stroop test successfully identified the cognitive benefits of Rifaximin for Covert HE. Therefore, clinicians should consider integrating digital or smartphone-based versions of the Stroop test into routine practice to identify and monitor patients who may benefit from rifaximin before they experience a major decompensation event.
