High HbA1c levels before surgery often indicate a higher risk of systemic and local complications. Chronic hyperglycemia leads to microvascular damage, which may impair the eye's ability to heal after invasive surgery. Additionally, patients with uncontrolled sugar are more prone to postoperative inflammation and vitreous hemorrhage. Therefore, clinicians must emphasize the importance of reaching a stable metabolic state before proceeding with elective vitreoretinal interventions. This proactive approach helps in maximizing the functional benefits of the surgery for the patient.
Successful management requires a multidisciplinary strategy involving ophthalmologists and endocrinologists. Specifically, stabilizing blood sugar levels helps reduce the likelihood of neovascular glaucoma and persistent edema. However, surgeons must also be cautious of rapid glycemic drops just before surgery, as some studies suggest this may trigger transient worsening of retinopathy. Ultimately, tailored optimization ensures that the delicate retinal tissues have the best environment for recovery.
Most guidelines suggest a target HbA1c level below 7% to 8% to minimize the risk of postoperative complications and ensure better visual recovery.
