Paramedic skill sets play a crucial role in determining whether older residents in aged care facilities require transport to the hospital. A recent retrospective cohort study analyzed over 23,000 ambulance attendances in Tasmania to identify the primary drivers of these transport decisions. The findings highlight a significant association between clinical severity and the specific expertise of the attending emergency crew. Consequently, understanding these predictors can help optimize healthcare resource allocation for the elderly population.
Researchers utilized gradient boosting models and multivariable logistic regression to evaluate routine electronic records from 2018 to 2024. They found that while acute illness severity, measured by NEWS2 scores and respiratory rates, strongly predicted transport, the type of paramedic crew functioned as an independent factor. Specifically, residents whom standard or intensive care crews attended were much more likely to face hospitalization compared to those whom specialized models managed.
The study revealed that Extended Care Paramedics (ECPs) demonstrated a dramatically lower probability of hospital transport. For instance, the adjusted transport probability for ECPs was only 0.50, whereas it exceeded 0.85 for standard and intensive care paramedic crews. Therefore, the researchers suggest that expanding the scope of paramedic practice could significantly reduce the burden on emergency departments. Moreover, these findings are particularly relevant for evolving healthcare systems, such as those in India, where optimizing pre-hospital care remains a high priority.
Additionally, the initial pain score and calendar month emerged as notable predictors of transport. However, the influence of the crew skill set remains the most modifiable factor identified in this research. By implementing extended-scope paramedic models, healthcare administrators can potentially improve the management of low-risk cases within the aged care facility itself. Furthermore, prospective evaluation of these models is now necessary to ensure they maintain patient safety while enhancing system efficiency. Specifically, the integration of ECPs could serve as a blueprint for geriatric emergency care globally.
