The Pain Medicine Fellowship Match has undergone a fundamental transformation between 2019 and 2025. Historically, anesthesiology graduates dominated the field, yet recent data reveals a dramatic 70% decline in applicants from this specialty. Conversely, interest from Physical Medicine and Rehabilitation (PM&R) and Emergency Medicine (EM) has surged significantly. This shift signifies a move toward a multidisciplinary workforce. Between 2019 and 2025, total applicants for the Pain Medicine Fellowship Match dropped from 520 to 442. This decline primarily stems from a sharp reduction in anesthesiology candidates, falling from 351 to 106 over the period. Meanwhile, PM&R applicants increased by 68%, reaching 170 in 2025. This growth makes PM&R the largest single specialty group in the current cycle. Furthermore, Emergency Medicine interest grew sixfold, rising from 10 to 61 applicants. Despite the shrinking overall pipeline, the fill rate improved from 82.3% to 85.4% in the last year. This stabilization suggests that newer specialties are effectively filling the vacancy gap left by anesthesiology.
Multiple factors drive these demographic changes. Experts suggest that rising compensation in primary residency specialties and declining pain reimbursement rates are major contributors. However, the diversifying applicant pool also provides unique opportunities for broader clinical perspectives in patient care. Because fellows now come from various backgrounds, curricula may require specific adjustments to ensure standardized competency. Strategic interventions, such as early mentorship and increased specialty exposure, remain vital. These efforts will help sustain the workforce and ensure patient access to high-quality interventional and chronic pain care across all regions.
