Recent data from the Dana-Farber Cancer Institute (DFCI) demonstrates the growing clinical significance of multi-cancer early detection tests. These liquid biopsies analyze circulating biomarkers to identify multiple malignancies simultaneously. As these technologies enter routine practice, clinicians must understand the diagnostic pathways and outcomes associated with positive test signals. The prospective case series evaluated patients between late 2023 and early 2025 to determine real-world performance metrics.
The study followed 14 patients with positive results from the Grail Galleri test. Notably, 78.6% of these participants received a confirmed cancer diagnosis, while 21.4% were identified as false positives. Specifically, 54.5% of confirmed cases involved solid tumors, and 45.5% were hematologic malignancies. Furthermore, the test demonstrated exceptional precision in identifying the cancer signal origin (CSO). Interestingly, over 90% of the detected malignancies currently lack standard screening guidelines. Consequently, these results suggest that multi-cancer early detection tests could fill critical gaps in traditional oncology screening.
Timely adjudication remains vital for patient outcomes and psychological well-being. Patients presented at the DFCI MCED clinic a median of 28 days after their test result. Subsequently, the multidisciplinary team achieved diagnostic resolution within a median of 20 days. Clinicians diagnosed six cancers at stage I or II, while five were found at stages III or IV. However, the study emphasizes that dedicated clinical programs are necessary to handle the complexity of positive signals. High diagnostic expertise ensures that patients receive quick and accurate evaluations following a positive screening.
