Chronic kidney disease remains a significant challenge for patients with diabetic nephropathy, especially those progressing to end-stage renal disease. Managing microinflammation in peritoneal dialysis has emerged as a critical factor for improving patient longevity and treatment quality. While peritoneal dialysis offers a flexible treatment option, persistent systemic inflammation often compromises its effectiveness. Recently, researchers developed a predictive model to identify high-risk patients by analyzing specific immune markers and their impact on dialysis efficacy.
In a prospective cohort study involving 236 patients, researchers categorized participants into high-inflammation and microinflammation groups. The results indicated that the high-inflammation group experienced significantly lower dialysis efficiency compared to those with lower inflammatory markers. Specifically, lower Kt/V values in these patients pointed toward a reduced ability to clear uremic toxins. Furthermore, patients with elevated inflammation markers faced higher infection rates and decreased overall survival. Therefore, identifying these inflammatory patterns early is vital for modifying treatment plans and improving patient prognosis. The predictive model built via logistic regression successfully identified key predictors of poor outcomes, offering clinicians a practical tool for risk stratification.
Addressing these inflammatory markers allows for more personalized care in diabetic nephropathy. For instance, early intervention with anti-inflammatory strategies or adjustments in the dialysis prescription may mitigate the adverse effects of chronic systemic stress. Clinicians should monitor immune markers regularly to ensure the dialysis remains effective and to prevent secondary complications associated with persistent microinflammation.
