Distinguishing between functional and organic memory issues often challenges clinicians in busy practice settings. Therefore, researchers recently investigated a specific behavioral marker to aid this differentiation. The long-term memory sign (LTMS) represents a neurologically unusual complaint pattern that frequently signals functional cognitive disorders (FCD). Specifically, this sign occurs when a patient reports that their long-term memory loss is equal to or worse than their short-term memory loss. This self-reported pattern differs significantly from the progressive, short-term-first decline typical of neurodegenerative conditions like Alzheimer’s disease.
Furthermore, a comprehensive study utilized standardized clinical interviews for 568 consecutive patients seeking neuropsychological evaluations. Consequently, the researchers divided these individuals into five distinct diagnostic categories, including dementia, mild neurocognitive disorder, and primary psychiatric conditions. Additionally, they assessed patients with subjective cognitive complaints and those with invalid cognitive profiles. The results indicated that the long-term memory sign demonstrated a remarkably high specificity of 92.2% for functional cognitive disorders. However, the sensitivity remained relatively low at 28.3%, suggesting it is best used to confirm rather than exclude a diagnosis.
In contrast, patients with confirmed dementia rarely exhibited this specific complaint pattern, with only 4.3% showing a positive sign. Conversely, nearly 44% of those with invalid cognitive presentations displayed the marker. Similarly, over a quarter of patients with primary psychiatric diagnoses reported these specific memory patterns. Moreover, clinicians can elicit this sign quickly during a routine interview by using two standardized prompts. Therefore, it serves as a powerful and efficient \"rule-in\" tool for functional disorders in various clinical settings.
A positive sign occurs when a patient claims their long-term memory loss is equal to or worse than their short-term memory loss. This pattern contradicts the typical temporal gradient of neurodegenerative diseases, where recent memories fade first.
While the sign has high specificity for functional disorders, its low sensitivity means a negative result cannot exclude neurodegeneration. Clinicians should use it as a supplementary diagnostic tool rather than a standalone screening test.
