In the Indian context, implementation faces unique hurdles. High tuberculosis prevalence often results in persistent lung lesions, which mimic malignancy on scans. Therefore, Indian experts emphasize the need for specialized algorithms that account for local epidemiology and bidi smoking habits. Additionally, the lack of a national screening framework means that costs and resource allocation remain significant barriers to equitable access. Establishing a successful program requires reaching the most vulnerable populations while maintaining adequate healthcare capacity.
The main benefit is a significant reduction in lung cancer-specific mortality, typically around 20% to 24% in high-risk populations. By detecting tumors at an earlier, treatable stage, LDCT allows for curative interventions that traditional X-rays might miss.
The primary risks include high false-positive rates, which can lead to unnecessary biopsies or surgeries. There is also a risk of overdiagnosis, where slow-growing cancers are detected that might not have otherwise affected the patient's lifespan or quality of life.
Implementation is often hindered by limited healthcare system capacity and the difficulty of reaching the target high-risk population.
