Hypervirulent Klebsiella pneumoniae (hvKp) has emerged as a formidable global health threat, particularly in regions like India where invasive infections are rising. This pathogen possesses unique virulence traits that allow it to bypass the natural protective barriers of the gut-liver axis. Consequently, it causes severe liver abscesses that can metastasize to distal organs such as the eyes and brain. Unlike classical strains, hvKp often targets healthy individuals, making the understanding of hvKp liver abscess pathogenesis critical for clinical management.
The journey of hvKp begins in the gastrointestinal tract. Here, the bacteria must overcome the host's microbiota and physical mucosal barriers. Research suggests that gut dysbiosis and local inflammation facilitate the translocation of these bacteria into the portal circulation. Specifically, the impairment of the intestinal barrier permits highly pathogenic strains to enter the bloodstream and seed the liver. Once established, these bacteria encounter formidable host immune responses.
The interaction between hvKp and Kupffer cells is a pivotal step in hvKp liver abscess pathogenesis. While Kupffer cells typically clear circulating bacteria, hvKp strains—especially those with K1 and K2 capsular types—resist phagocytosis. Furthermore, evidence suggests that hvKp can survive and even replicate within macrophages. This intracellular survival helps the bacteria evade the systemic immune response and contributes to the formation of pus-filled lesions. Therefore, the ability to resist macrophage-mediated clearance is a hallmark of hypervirulence.
Current murine models have provided significant insights into these processes. However, these models have limitations in fully replicating the human disease continuum. Researchers highlight that outcomes can be strain-specific and model-dependent, which may not always be generalizable to clinical practice. Addressing these knowledge gaps requires well-characterized hvKp strains and diverse experimental approaches. In addition, resolving conflicting findings through robust models is essential for advancing therapeutic research.
Diabetes mellitus is the most significant risk factor, as hyperglycemia impairs immune function and facilitates bacterial dissemination. Other factors include gut dysbiosis and the presence of K1/K2 serotypes.
Classical strains primarily cause opportunistic infections in hospitalized patients. In contrast, hvKp is more invasive, affects healthy individuals, and frequently leads to metastatic complications like endophthalmitis.
The gut-liver axis serves as the primary route for hvKp translocation. Disruptions in the intestinal barrier allow the bacteria to travel via the portal vein directly to the liver.
