Tumor necrosis factor-alpha (TNF-alpha) inhibitors remain central to managing inflammatory conditions like rheumatoid arthritis and ulcerative colitis. However, biologic therapy alters physiological immune surveillance. Therefore, clinicians must continuously review real-world postmarketing surveillance data. A comprehensive study recently investigated spontaneous safety reports to evaluate golimumab adverse events. This analysis examined 50,031 adverse drug event reports recorded between Q2 2009 and Q3 2025 in the FDA Adverse Event Reporting System. Consequently, the researchers identified significant safety signals that expand upon current prescribing guidelines.
Postmarketing pharmacovigilance captures vital safety evidence that pre-registration clinical trials often overlook. Clinical trials feature narrow enrollment criteria, short observation periods, and limited sample sizes. In contrast, the FDA Adverse Event Reporting System (FAERS) collects broad real-world data across heterogeneous populations globally. In this retrospective analysis, researchers mined 50,031 reports where clinicians identified golimumab as the primary suspect drug.
To maximize statistical accuracy, the investigators implemented four distinct disproportionality algorithms. Specifically, they utilized the reporting odds ratio, proportional reporting ratio, Bayesian confidence propagation neural network, and multi-item gamma Poisson shrinker. By demanding concordance across these four analytical algorithms, the authors minimized statistical noise and false discoveries. Ultimately, the data screening pipeline identified 397 positive safety signals across 25 distinct system organ classes. These concordant metrics highlight meaningful statistical disproportion in adverse reaction reporting. Thus, this extensive dataset provides clinicians with a robust map of therapeutic risks in routine practice.
Concordant signals across all four algorithms represent the most reliable drug safety signals in disproportionality analyses. In this evaluation, concordant positive signals concentrated primarily within infections and infestations, alongside surgical and medical procedures. Because golimumab potently inhibits TNF-alpha, infectious complications formed the largest cluster of reported adverse events.
Respiratory tract infections showed the highest reporting frequencies throughout the dataset. In particular, pneumonia and lower respiratory tract infections generated powerful statistical signals across all algorithms. Furthermore, clinicians submitted frequent reports of influenza, nasopharyngitis, and acute sinusitis. Cutaneous tissues also exhibited marked vulnerability, with cellulitis appearing as a major signal. Additionally, disproportionality signals in surgical and medical procedures reflected secondary interventions required to manage severe tissue damage. For instance, severe bacterial infections and refractory deep tissue abscesses frequently required operative drainage and wound debridement. Therefore, physicians must maintain vigilance when evaluating biologic recipients presenting with common febrile or localized respiratory symptoms.
While product labels thoroughly discuss respiratory tract infections, this database analysis exposed several previously undocumented infectious complications. Specifically, the algorithms detected positive reporting signals for urinary tract infections and ear infections. These unexpected findings suggest that anti-TNF therapy impairs localized barrier defense across varied anatomical sites.
Most remarkably, the disproportionality screen revealed strong positive signals for vector-borne tropical infections. Notably, these events included flavivirus infection, Zika virus infection, and Dengue fever. Prescribing information currently omits these tropical arboviral pathogens entirely. However, tumor necrosis factor plays a crucial biological role in controlling viral replication and clearing intracellular viral loads. When golimumab suppresses this protective pathway, patients lose essential cytokine defense mechanisms. Consequently, immunosuppressed individuals living in or visiting tropical regions face heightened danger from endemic arboviruses. Clinicians in endemic regions like India must actively recognize this risk. Furthermore, medical practitioners should educate patients on personal mosquito avoidance measures and evaluate acute febrile episodes promptly.
Tumor necrosis factor contributes significantly to physiological tumor surveillance and the natural destruction of malignant cells. Therefore, ongoing clinical concern persists regarding whether long-term anti-TNF therapy facilitates de novo neoplastic growth. In this FAERS analysis, researchers detected 29 positive malignancy signals that currently do not appear in official prescribing documents.
Among these newly detected signals, basal cell carcinoma demonstrated a prominent disproportional reporting pattern. Because cutaneous malignancies occur more frequently in immunosuppressed hosts, clinicians must ensure regular dermatological examinations. Furthermore, the algorithms identified unexpected signals for common solid tumors, including colorectal cancer, cervical cancer, and breast cancer. Although disproportionality metrics cannot establish direct biological causality, these safety signals warrant prudent clinical consideration. Confounding elements like older patient age and prior immunosuppressant exposure undoubtedly influence baseline oncological risks. Nevertheless, detecting nearly thirty unlisted cancer signals emphasizes the value of preventive screening. Clinicians must confirm that patients complete routine age-appropriate cancer screenings prior to and during golimumab therapy.
Evaluating time-to-onset distributions helps healthcare teams anticipate high-risk periods during biologic therapy. In this study, temporal analysis revealed that the incidence of adverse drug events peaked sharply within the first few months of treatment initiation. Subsequently, the risk settled into a lower, stable plateau during long-term maintenance therapy.
This early vulnerability reflects the rapid immunological shift that occurs upon biologic induction. Because golimumab quickly neutralizes circulating and transmembrane TNF-alpha, host immune defenses experience acute suppression. Consequently, latent pathogens can reactivate rapidly, and nascent infections often become clinically overt during this transitional phase. Meanwhile, clinicians must structure intensive monitoring schedules during the first ninety days of treatment. Physicians should order baseline blood counts, liver enzymes, and renal function panels while tracking systemic infection symptoms. Moreover, patient education should stress prompt reporting of fevers, persistent coughs, or new skin lesions immediately. Proactive surveillance enables early clinical intervention and prevents minor adverse reactions from escalating into dangerous medical emergencies.
Postmarketing surveillance reveals that respiratory tract infections represent the most frequent complications in patients receiving golimumab therapy. Specifically, reported cases frequently involve pneumonia, lower respiratory tract infection, influenza, nasopharyngitis, and sinusitis. In addition, cutaneous infections such as cellulitis appear with notable frequency. Because tumour necrosis factor alpha plays an indispensable role in mucosal host defense and granuloma formation, systemic blockade significantly impairs localized pathogen clearance across vulnerable tissue compartments.
Golimumab exerts potent immunosuppressive effects by binding to soluble and transmembrane tumor necrosis factor alpha. Consequently, therapy disrupts cytokine signaling networks essential for innate immune responses against intracellular pathogens. Although prescribing labels historically emphasized bacterial and mycobacterial pathogens, sustained immune neutralization impairs interferon cascades and cell-mediated antiviral clearance. Therefore, vulnerable hosts living in endemic environments face elevated susceptibility to severe mosquito-borne arboviral diseases, including flavivirus infections, dengue fever, and Zika virus complications.
Data mining demonstrates that adverse events emerge most frequently within the initial months of starting golimumab. Specifically, heightened vulnerability occurs during the initial treatment induction phase, when rapid cytokine suppression drastically alters immune equilibrium. However, the cumulative risk continues throughout chronic therapy, necessitating vigilant baseline evaluation and persistent surveillance. Therefore, clinicians must maintain rigorous monitoring protocols immediately upon initiation, checking for emerging fever, persistent respiratory complaints, abnormal cytopenias, and novel dermatological lesions.
