A recent retrospective study validated a new model for furcation involvement risk prediction in molars receiving fixed prostheses. Initially, this research followed 203 molars over five years to track the development of furcation involvement (FI). The findings highlight that clinical success relies on thorough periodontal assessment before crown placement.
The study identified periodontitis history as a major predictor. Specifically, patients with advanced stages of the disease showed a three-fold increase in risk. Additionally, anatomical features like root trunk length significantly influenced the outcome. Furthermore, molars with short root trunks demonstrated a much higher vulnerability across all follow-up intervals. Because the model performed best at the 3-year mark, it offers a reliable window for clinical intervention. However, the risk continues to rise steadily up to five years.
Consequently, clinicians should integrate this assessment into their routine prosthetic planning. Therefore, identifying high-risk molars allows for better patient communication and customized maintenance schedules. Moreover, these tools help in selecting the most appropriate restorative materials and designs. In addition, this proactive approach ensures long-term tooth survival and improves overall oral health outcomes.
A history of periodontitis and a short root trunk are the most significant predictors. Specifically, patients with Stage III or IV periodontitis are particularly vulnerable to increased bone loss in the furcation area.
The model demonstrated high accuracy, particularly at the three-year mark with an AUC of 0.81. This indicates strong sensitivity and specificity, making it a robust tool for clinical decision-making.
