The management of endometrial cancer has transitioned from a purely histopathological model to a sophisticated framework based on molecular precision. This shift is primarily centered on the integration of immune checkpoint inhibitors. Consequently, clinicians have observed a radical change in standards for different genomic subsets. For patients with mismatch repair deficient or microsatellite instability-high tumors, frontline immunotherapy endometrial cancer strategies have officially moved from late-line options to the primary standard of care.
Landmark results from the RUBY and NRG-GY018 trials have redefined the first-line setting. These studies demonstrate that combining dostarlimab or pembrolizumab with platinum-based chemotherapy significantly improves survival in patients with untreated advanced disease. Furthermore, the updated long-term data from these trials continue to show a sustained benefit across molecular subgroups. While the most dramatic responses occur in mismatch repair deficient populations, researchers are also identifying meaningful gains in proficient cohorts.
