Evolocumab in high-risk diabetes significantly lowers the danger of experiencing a first cardiovascular event. This finding comes from the VESALIUS-CV trial, which researchers presented at the American Diabetes Association (ADA) 86th Scientific Sessions in June 2026. The study specifically investigated patients with high-risk diabetes who had no history of heart attack or stroke. Consequently, these results provide a crucial roadmap for primary prevention in diabetic populations. Furthermore, most of these patients already used high-intensity statins, yet evolocumab offered substantial additional protection because it targets PCSK9 effectively.
The trial enrolled over 6,000 patients with high-risk diabetes defined by microvascular disease, insulin use, or long-standing disease duration. Researchers randomized participants to receive either evolocumab 140 mg or a placebo every two weeks. Additionally, the team monitored outcomes over a median of 4.6 years. The results showed a 29% reduction in the risk of death from coronary heart disease, myocardial infarction, or ischemic stroke. Thus, the drug demonstrated clear efficacy in preventing the first occurrence of major adverse cardiovascular events.
The VESALIUS-CV trial highlights that early and intensive LDL-C reduction is vital for patients with high-risk diabetes. While statins remain the first-line therapy, many patients do not achieve optimal targets alone. Moreover, the trial achieved a median LDL-C of 45 mg/dL in the treatment arm. This level is significantly lower than the 106 mg/dL observed in the placebo group. Therefore, adding a PCSK9 inhibitor like evolocumab can bridge the gap in cardiovascular protection for primary prevention.
In India, where the burden of diabetic dyslipidemia is exceptionally high, these findings resonate deeply with local practice. Local guidelines from the Lipid Association of India (LAI) already suggest aggressive lipid management for very high-risk individuals. Specifically, they recommend reaching LDL-C targets below 50 mg/dL or even 30 mg/dL in certain cases. Furthermore, the safety profile of evolocumab in this study remained consistent with previous large-scale trials. There were no new safety concerns although patients reached very low LDL-C levels during the treatment period.
Ultimately, these data support a shift toward more proactive lipid management in primary prevention for diabetes. Waiting for a first cardiovascular event before intensifying therapy may no longer be the best approach for clinical success. Instead, clinicians should consider PCSK9 inhibitors earlier for their high-risk diabetic patients because this strategy substantially improves long-term outcomes.
