Enteric infections in children under five remain a critical focus for global health, representing a leading cause of preventable death. A recent analysis using Global Burden of Disease (GBD) 2021 data highlights both remarkable progress and significant challenges. While global mortality has declined by over 77% since 1990, substantial disparities persist between different socioeconomic regions. Consequently, clinicians and policymakers must address these deep-seated inequities to protect the most vulnerable populations.
Researchers analyzed data across 204 countries, evaluating incidence, mortality, and disability-adjusted life years (DALYs). In 2021, enteric infections caused approximately 63.4 deaths per 100,000 children globally. However, the burden remains heavily concentrated in specific hotspots. For instance, low-SDI (Socio-demographic Index) regions experienced mortality rates 166-fold higher than high-SDI regions. Diarrheal diseases accounted for the vast majority of these deaths, representing over 80% of the total mortality burden.
The study identifies specific environmental and nutritional factors that drive high disease rates. Unsafe water, inadequate sanitation, and malnutrition collectively contributed to 82.3% of attributable DALYs. Furthermore, neonates face the highest incidence rates, and male mortality consistently exceeds female mortality across various regions. Geographically, sub-Saharan Africa and South Asia, including India, remain the most affected areas. Therefore, multisectoral interventions focusing on WASH (water, sanitation, and hygiene) and nutritional support are essential for sustainable improvement.
The leading causes are diarrheal diseases and invasive non-typhoidal Salmonella (iNTS). These infections often result from exposure to contaminated water, poor sanitation, and are exacerbated by underlying malnutrition.
Low-SDI regions often lack robust healthcare infrastructure and access to clean water. Consequently, children in these areas face higher exposure to pathogens and have fewer resources to manage severe dehydration or systemic infections.
Global incidence and mortality have declined by 68.6% and 77.7%, respectively, since 1990. Despite this progress, the relative gap between high-income and low-income regions has widened in some metrics, necessitating targeted global health focus.
