Degenerative Cervical Myelopathy symptoms often manifest as a subtle prodrome of pain long before significant neurological deficits appear. Chronic spinal cord compression causes this progressive condition, which clinicians often miss due to nonspecific early warning signs. Consequently, healthcare providers must understand the chronology of these symptoms to facilitate timely intervention and prevent irreversible damage.
The study analyzed 138 patients to identify the most frequent early indicators. Notably, neck pain and back pain emerged as the most common symptoms, occurring in 84.1% and 72.5% of participants, respectively. Furthermore, many patients recalled back pain (40.6%) and neck pain (31.9%) as the very first symptoms they experienced. These pain-related markers often persist for years before more recognizable signs, such as hand incoordination or gait imbalance, develop. Specifically, the mean duration for back pain reached approximately 8.6 years, whereas upper extremity (UE) pain lasted about 6.2 years. This long lead time suggests that chronic musculoskeletal pain in older adults warrants a high index of suspicion for underlying myelopathy.
Symptoms in DCM rarely occur in isolation. For instance, the co-occurrence of neck and back pain occurred in 65.2% of cases. Additionally, the combination of neck pain with UE numbness (57.2%) or gait imbalance (50.7%) is highly frequent. Although symptoms like saddle numbness and UE weakness are less common initially, patients often rate them as the most bothersome. Therefore, clinicians must look beyond isolated complaints and evaluate the patient for a constellation of sensory and motor changes. Prompt referral to a spine specialist remains the gold standard once these patterns are recognized.
