A recent retrospective case series published in the Australian Veterinary Journal explores the diagnostic utility of CT imaging of mycobacterial lesions. The study focused on canine leproid granuloma syndrome (CLGS). This condition involves chronic cutaneous and subcutaneous mycobacterial infections. Therefore, researchers aimed to describe the specific imaging features that could aid in better clinical assessment.
The study reviewed three confirmed cases of cutaneous mycobacterial disease in dogs. These included a Dogue de Bordeaux, a Whippet cross, and an English Springer Spaniel. Initially, all dogs presented with chronic multifocal lesions. These lesions were notably unresponsive to standard antimicrobial therapies. Consequently, CT imaging of mycobacterial lesions was performed to visualize the internal extent of the disease before surgical biopsy.
Specifically, all three cases exhibited multifocal soft tissue nodules. These nodules were located within the cutaneous and subcutaneous layers. A characteristic finding across all cases was peripheral rim enhancement following contrast administration. Furthermore, two cases showed visible cutaneous defects and regional lymphadenomegaly. One case even displayed multifocal pulmonary nodules, which clinicians presumed were mycobacterial granulomas. Fortunately, appropriate antibiotic therapy led to clinical resolution in two cases within five months.
Cutaneous mycobacterial infections often mimic other dermatological diseases. However, this study suggests that CT is a highly useful modality. It allows clinicians to assess the full extent of tissue involvement. Moreover, it identifies deep-seated nodules that may be missed during a physical examination. Because these infections can be underdiagnosed, increased awareness of these imaging patterns is essential for veterinary and infectious disease specialists.
In addition, the use of Mycobacterium PCR testing remains the gold standard for confirmation. However, the anatomical detail provided by CT assists in planning surgical biopsies. It also helps in monitoring the response to long-term therapy. Thus, integrating advanced imaging into the diagnostic protocol can improve outcomes for patients with suspected granulomatous disease.

CT imaging of mycobacterial lesions helps determine the depth and spread of the infection. This assessment is vital for planning surgical interventions and monitoring the effectiveness of antibiotic treatment over several months.
It is often considered rare or underdiagnosed. This is due to its similarity to other cutaneous diseases. However, awareness is growing as molecular testing and advanced imaging become more accessible.
