Managing spinal pathology in Neurofibromatosis type 1 (NF1) requires vigilant monitoring due to the potential for insidious progression. Initially, clinicians may observe asymptomatic lesions. However, a recent illustrative case describes a 43-year-old woman who experienced significant neurological decline due to a massive, progressive Cervicothoracic meningocele in NF1. Because this lesion was managed conservatively for over a decade, it eventually caused severe neurovascular compression and complex spinal deformity.
Over an 11-year observation period, serial imaging revealed a dramatic transformation of the lateral meningocele. Specifically, the lesion eventually exceeded 10 cm in size. Notably, the expansion resulted in severe kyphoscoliosis, extensive vertebral scalloping, and widened neural foramina. Consequently, the meningocele deviated the spinal cord and displaced the vertebral and carotid arteries. Additionally, the patient suffered from anterior compression of the esophagus and airway. Therefore, she presented with worsening gait imbalance, left upper limb weakness, and dysesthesia. Because these symptoms correlated with compression at the C5-T1 levels, immediate intervention became necessary.
Given the progressive neurological deficits and life-threatening neurovascular compromise, surgeons opted for operative intervention. Specifically, they performed a surgical repair of the anterior cervical meningocele to alleviate the mass effect. Meanwhile, postoperative imaging confirmed marked radiological improvement. Furthermore, the patient experienced significant relief from the debilitating symptoms. This success emphasizes that while these lesions may follow an indolent course, early recognition of clinical deterioration is vital. Therefore, physicians must remain alert to subtle changes during follow-up visits.
This case underscores several critical management pearls for NF1 patients. Firstly, spinal meningoceles can enlarge progressively even after years of stability. Consequently, clinicians must provide long-term radiological surveillance for all patients. Secondly, cervical involvement poses unique risks for airway and vascular compression. Moreover, a multidisciplinary approach is essential when spinal pathology coexists with other central nervous system tumors. For instance, this patient also presented with bilateral optic pathway gliomas, suggesting a more extensive phenotype. Ultimately, timely surgical reassessment can prevent irreversible neurological damage.
Progression often occurs due to dural ectasia and inherent dural weakness associated with NF1. Chronic cerebrospinal fluid (CSF) pulsations against these weakened areas can lead to gradual expansion, bone erosion, and subsequent compression of adjacent structures.
Surveillance is vital because these lesions can remain asymptomatic for years before suddenly progressing. Continuous monitoring through serial imaging allows clinicians to detect neurovascular or aerodigestive compression early, facilitating timely surgical intervention.
