A recent prospective multicenter study has identified a critical link between cerebral small-vessel disease delirium risk and elderly patients undergoing non-cardiac surgery. Postoperative delirium (POD) remains a frequent and severe complication in the geriatric population. It often leads to prolonged hospital stays, cognitive decline, and increased mortality. Consequently, the researchers monitored the patients for delirium twice daily for three days following surgery.
The results revealed a clear dose-response relationship. Specifically, the risk of delirium rose progressively as the burden of cerebral small-vessel disease increased. In the most severely affected patients, the incidence of delirium reached 28%. Patients with a high disease burden faced nearly double the risk of delirium compared to those with lower scores. Notably, the presence of lacunes alone was associated with a nearly threefold increase in risk. Furthermore, the study estimated that almost half of all delirium cases in this group were potentially attributable to high pre-existing vascular disease levels.
These findings emphasize the importance of brain health assessment before surgery. Therefore, clinicians should consider preoperative neuroimaging to identify high-risk individuals. Early detection allows for the implementation of targeted delirium prevention strategies. Additionally, understanding a patient's neurovascular status can help set realistic expectations for postoperative recovery. Identifying these markers provides a valuable window for intervention in the aging surgical population.
