Clinicians often prioritize the aesthetic outcome when planning immediate implant placement in the anterior maxilla. Achieving the ideal screw channel emergence is difficult because the bone anatomy often dictates a labial inclination. Recently, a radiographic study utilized CBCT to assess the feasibility of angled screw channel systems during virtual planning. This research evaluated 295 sites across central incisors, lateral incisors, and canines to compare access channel angulations.
The results showed significant variability among different tooth types. For instance, canine sites demonstrated the highest percentage of straight access channels at 51%. Conversely, lateral incisors proved to be the most challenging, with only 5.1% allowing for straight access. Furthermore, the study observed a significant increase in access through the cingulum when using angled screw channel systems. These systems allow for angular corrections ranging from 5 to 25 degrees to improve prosthetic emergence.
Proper planning with CBCT is essential for identifying these restorative needs early. However, clinicians must remember that lateral incisors frequently require more extreme angulation corrections. Therefore, understanding these probabilities helps dentists manage patient expectations and technical requirements during surgery. Overall, the transition from straight to angled access significantly enhances the probability of a successful screw-retained restoration while maintaining the integrity of the incisal edge.
