The study utilized a rat model to evaluate kidney function after amikacin exposure. Results showed that amikacin significantly elevated serum creatinine and blood urea nitrogen levels. In contrast, Roflumilast co-treatment restored these markers toward baseline. Furthermore, the drug activated the Nrf2/HO-1 signaling pathway. This activation is vital because it upregulates antioxidant enzymes like SOD and catalase. Consequently, the drug successfully neutralized lipid peroxidation and oxidative stress.
Amikacin typically triggers a severe inflammatory response through the NF-κB cascade. Moreover, Roflumilast co-administration suppressed this pathway, leading to lower levels of cytokines like TNF-α and IL-1β. In addition, the treatment modulated apoptotic markers. It reduced pro-apoptotic proteins like Bax and Caspase-3 while increasing anti-apoptotic Bcl-2 expression. Histopathological evaluations confirmed these biochemical findings. Therefore, Roflumilast could become a vital therapeutic strategy for patients requiring aminoglycoside therapy.
