A single-center, triple-blind, randomized, placebo-controlled pilot trial evaluated the effect of citicoline on cognitive and functional recovery in 60 adults with mild-to-moderate traumatic brain injury. Patients received citicoline 1000 mg/day or placebo for 90 days. At day 90, patients receiving citicoline demonstrated significantly better cognitive and functional outcomes. Mean MMSE scores were 25.2 versus 21.5 with placebo, while Barthel Index scores were 85.0 versus 70.0 and GOS-E scores were 5.2 versus 3.6; all comparisons had P<0.001. New or worsening intracerebral hemorrhage occurred in 2 patients receiving citicoline and 3 receiving placebo, with no significant difference between groups. The findings suggest that citicoline may improve cognitive and functional recovery following mild-to-moderate TBI, although the authors emphasize that larger multicenter trials are required to confirm these preliminary findings.
