This retrospective NSQIP study evaluated 13,716 adults undergoing craniotomy for supratentorial malignant brain tumors, comparing 8,830 patients before COVID-19 (2016–2019) with 4,886 during the pandemic (2020–2021). Thirty-day mortality was 3.1% pre-pandemic versus 2.6% during COVID-19 (P=0.087). After adjustment, pandemic-period surgery was associated with lower odds of 30-day mortality (OR 0.70; 95% CI 0.55–0.88; P=0.0027). Age, disseminated cancer and postoperative pneumonia, prolonged ventilation and stroke remained significant mortality-associated factors.
