Spinal cord injury (SCI) typically leads to profound muscle atrophy, aerobic deconditioning, and metabolic dysfunction. While standard exercise interventions provide some benefit, their effects are often modest. Consequently, researchers have looked for ways to amplify the body\'s physiological response to physical activity. A recent randomized controlled trial explored whether testosterone augmented exercise SCI protocols could safely improve outcomes for this population.



Impact of Testosterone Augmented Exercise SCI on Body Composition


The study results demonstrated a significant advantage for the multimodality intervention. Participants who received intramuscular testosterone undecanoate alongside functional electrical stimulation-assisted leg cycling (FES-LC) and arm ergometry (AE) gained substantially more lean mass. Specifically, the multimodality group showed a whole-body lean mass increase of 1.84 kg. Moreover, they gained 0.92 kg of lean mass in the lower extremities, which is a critical area often resistant to traditional rehabilitation. These findings suggest that testosterone plays a vital role in counteracting the muscle wasting associated with chronic SCI.



Cardiometabolic Markers and Aerobic Capacity


In addition to muscle gains, the testosterone-augmented group experienced significant improvements in blood markers. The intervention corrected anemia in a higher proportion of participants compared to the placebo group. While between-group differences in aerobic capacity (peak VO2) did not reach statistical significance, the multimodality group showed a notable 19% within-group improvement. Furthermore, this dual approach proved safe, as adverse event rates were similar across both study arms. Therefore, combining hormonal therapy with multimodal exercise could offer a more comprehensive strategy for improving the health of individuals living with SCI.
