Cardiac resynchronization therapy (CRT) remains a cornerstone in managing patients with heart failure and reduced ejection fraction. Despite its proven efficacy, clinicians often face the challenge of "non-responders," where nearly one-third of patients do not show symptomatic or structural improvement. Recent research highlights the SII and CRT response connection, suggesting that systemic inflammation plays a pivotal role in determining therapeutic success. The Systemic Immune-Inflammation Index (SII) serves as a composite biomarker reflecting the balance between pro-inflammatory and immune-regulatory pathways.
Research indicates that patients who do not respond to CRT often exhibit significantly higher baseline SII levels. This index, calculated using platelet, neutrophil, and lymphocyte counts, provides a more comprehensive picture of systemic inflammation than traditional markers like C-reactive protein (CRP). Furthermore, while responders often maintain stable inflammatory levels, non-responders may show a significant increase in SII during follow-up. Consequently, an elevated baseline SII may signal an unfavorable biological environment that impairs myocardial recovery.
Integrating the SII and CRT response data into clinical practice could refine patient selection. Studies have identified chronic ischemic heart disease and high baseline SII as independent predictors of poor response. By utilizing this readily available laboratory parameter, cardiologists can better stratify risks. They can identify patients who might require more intensive anti-inflammatory management or alternative device strategies. Although the discriminatory ability is currently moderate, SII offers a cost-effective tool for enhancing clinical decision-making in heart failure care.
The Systemic Immune-Inflammation Index provides valuable insights into the persistent inflammatory activity that hinders cardiac recovery. While it may not be a direct cause of non-response, it reflects the complex biological milieu of the heart failure patient. Identifying high-risk individuals through this simple biomarker could pave the way for more personalized treatment approaches and better resource allocation in cardiac care.
SII is a biomarker calculated by multiplying the platelet count by the neutrophil count and dividing the result by the lymphocyte count. It reflects a patient's integrated inflammatory and immune status more accurately than single-cell counts.
Elevated SII levels are associated with a higher likelihood of non-response to cardiac resynchronization therapy. This helps doctors identify which patients are less likely to experience heart remodeling after the procedure.
While baseline CRP levels may be similar between responders and non-responders, SII has shown a stronger independent association with the therapy's success. It also tracks persistent inflammation during the follow-up period.
