Acute kidney injury (AKI) frequently occurs in patients receiving anti-PD-L1 immunotherapy. This clinical observation suggests that PD-L1 maintains renal homeostasis. However, the exact mechanism remained unknown until now. A groundbreaking study recently identified a T-cell independent role for PD-L1 in AKI repair. Specifically, the research focuses on how PD-L1 interacts with the BRCA1 protein to safeguard tubular epithelial cells (TECs).

To translate these findings, the team developed a CGA-functionalized extracellular vesicle delivery system. This platform provides targeted delivery of PD-L1 specifically to injured TECs. Additionally, the system effectively restored PD-L1 levels in both PD-L1 knockdown and T-cell knockout mouse models. Thus, PD-L1 supplementation represents a promising therapeutic strategy for treating AKI. Moreover, this pathway operates independently of the canonical immunomodulatory functions associated with T cells.
