China and India share a significant challenge: a high prevalence of mental disorders coupled with a substantial treatment gap. While digital health offers a solution, understanding online psychotherapy willingness among patients remains critical for successful implementation. A recent multicenter cross-sectional survey published in JMIR Formative Research sheds light on how patients perceive digital interventions and the barriers that hinder their adoption.



Factors Driving Online Psychotherapy Willingness


The study surveyed 361 eligible participants and found a mean willingness score of 70 out of 100. This relatively high score suggests a strong baseline interest in digital care. Interestingly, 86.4% of patients expressed a preference for short-term therapy, typically consisting of one to ten sessions. Furthermore, cost remains a pivotal factor for most individuals. Over 92% of participants expected the cost per session to stay below approximately US $55.50.



Patients identified convenience and perceived anonymity as the primary benefits of seeking care online. For many, the ability to receive treatment without the fear of being seen at a clinic reduces the weight of social stigma. Additionally, the regression analysis highlighted that younger age and positive past experiences with online platforms significantly boost online psychotherapy willingness. Conversely, high levels of self-stigma act as a powerful deterrent, even when patients recognize the practical benefits of digital access.



Addressing the Barriers to Digital Care


Despite the high interest, several roadblocks persist in the transition to virtual mental health services. The leading concern reported by participants was data security and privacy. Because mental health data is highly sensitive, patients fear that potential breaches could lead to social or professional repercussions. Furthermore, nearly 20% of participants expressed difficulty in establishing a therapeutic rapport through a screen, missing the nuances of face-to-face interaction.



Clinicians must also consider that satisfaction with current psychiatric medications and previous online interventions influences future engagement. Therefore, integrating digital therapy as a tailored supplement rather than a generic replacement is essential. Infrastructure investment and robust stigma-reduction programs will be vital for healthcare systems to bridge the existing treatment gap effectively.
