While type 2 diabetes mellitus is a known risk factor for Parkinson’s disease, selecting the right antidiabetic agent remains crucial for long-term health. Recent data explores the Metformin parkinsonism risk to determine if its neuroprotective potential in lab models translates to human patients. Researchers utilized the TriNetX global network to conduct a comprehensive retrospective cohort study. They matched over 75,000 metformin initiators with an equal number of dipeptidyl peptidase-4 inhibitor (DPP-4i) users. Using a 1-year landmark period, the team successfully minimized immortal time bias. Consequently, the primary analysis revealed that the overall risk of parkinsonism was similar between groups.
However, the study highlighted significant differences in other clinical outcomes. Metformin users demonstrated a 25% lower risk of all-cause mortality compared to those using DPP-4 inhibitors. Furthermore, an exploratory analysis focused on individuals surviving beyond 10 years of therapy. Additionally, the observed risk of parkinsonism was lower in this specific long-term survival group. Nevertheless, clinicians should interpret these long-term findings as hypothesis-generating rather than definitive. This occurs because such retrospective data can be affected by survivor or collider bias.
Therefore, while metformin remains a foundational treatment, its specific role in preventing neurodegeneration requires further prospective validation. Clinicians should focus on its established metabolic and survival benefits while monitoring for neurologic changes. Future research may clarify if the potential neuroprotective effects are dose-dependent or specific to certain patient subgroups.
