Maternal RSV immunization represents a breakthrough strategy to protect neonates during their most vulnerable first months. The scalability of this approach depends on leveraging existing antenatal care (ANC) platforms. Since most low- and middle-income countries (LMICs) already have established programs for tetanus and influenza vaccination, integrating maternal RSV immunization is feasible. However, success requires addressing challenges like cold chain logistics and equitable vaccine distribution. Moreover, modeling studies suggest that this strategy could avert nearly 30% of RSV-related deaths in these settings, provided that antenatal care coverage is optimized.
While safety findings are generally favorable, clinicians must remain vigilant regarding birth outcomes. Early trials with certain candidates noted a potential signal for preterm births; however, the currently prequalified vaccines maintain a strong safety profile when administered in the recommended window. To mitigate risks, the current FDA and WHO recommendations focus on administration between 32 and 36 weeks of gestation. Therefore, continuous post-licensure surveillance is essential to ensure long-term maternal and neonatal safety across diverse global populations.
