Recent research offers a detailed look at localized synovial sarcoma outcomes through a multi-center retrospective analysis. Researchers assessed 248 patients treated between 2000 and 2024 at three US quaternary medical centers. Consequently, this study provides meaningful data on prognostic factors for clinicians managing this rare soft tissue malignancy. Notably, the median age at diagnosis was 35 years, reflecting the disease's tendency to affect young adults and adolescents.
The multivariable analysis revealed that tumor size and surgical margin status are the strongest predictors of survival. Specifically, larger tumors correlated with significantly worse disease-free survival. Moreover, patients with positive surgical margins faced a higher risk of recurrence. Although nearly 70% of patients received perioperative radiotherapy, its direct impact on overall survival in this cohort was not statistically significant in multivariable models. Similarly, chemotherapy did not show a definitive survival benefit for these specific patients. Therefore, surgical precision remains the cornerstone of effective management in synovial sarcoma.
Clinicians often use perioperative chemotherapy and radiotherapy to manage localized disease. In this study, 60% of patients underwent chemotherapy, while nearly 70% received radiotherapy. However, the data suggests that these treatments did not significantly alter the disease-free survival or overall survival when researchers adjusted for tumor size and margins. Furthermore, the median disease-free survival reached 110.3 months. In addition, the study recorded a median follow-up of 5.2 years, providing a robust window for observing long-term outcomes. Consequently, personalized treatment plans must prioritize achieving negative margins and monitoring high-risk tumor characteristics.
