Pragmatic clinical trials (ePCTs) represent a significant shift toward research that reflects real-world clinical practice. By embedding studies within routine care, researchers can evaluate how interventions perform in diverse, everyday environments. A recent cross-sectional study from the NIH Pragmatic Trials Collaboratory investigated how these trials collect implementation data. Specifically, the team examined metrics like patient reach, clinician adoption, and long-term sustainability to determine the current landscape of implementation science within the Collaboratory.
The study results revealed that most research teams successfully track patient-centered metrics. For instance, 86% of the 32 trials measured patient reach, while 93% monitored fidelity to ensure the intervention occurred as intended. These high numbers suggest that the foundational elements of trial delivery are well-established. However, the data also highlighted critical areas where measurement remains sparse and inconsistent across different study phases.
Researchers noted significant gaps in system-level tracking within pragmatic clinical trials. Clinician adoption was measured by only 45% of the teams, and sustainability—the potential for an intervention to continue—was assessed by only 24%. Furthermore, actual sustainment after trial completion appeared in only 38% of the studies. Notably, only 31% of the trials tracked the costs associated with implementation. Therefore, the researchers emphasized that these missing data points are crucial for health system leaders deciding whether to adopt new practices.
Addressing these gaps is essential for the future of evidence-based medicine. When trials fail to measure adoption and costs, health systems may struggle to scale successful interventions effectively. Consequently, future ePCTs should prioritize these metrics to improve implementation strategies. Additionally, integrating these assessments will ensure that interventions are not only effective but also practical for the long term. Similarly, this approach helps bridge the gap between clinical research and routine practice, leading to more equitable healthcare improvements.
Many researchers focus primarily on immediate patient outcomes and intervention fidelity. Tracking long-term sustainability and detailed implementation costs requires extended resources and complex data collection methods that may not be available during the standard trial lifecycle.
The RE-AIM framework (Reach, Effectiveness, Adoption, Implementation, and Maintenance) provides a structured way to evaluate the public health impact of an intervention. This study specifically used RE-AIM constructs to identify which implementation outcomes researchers are currently prioritizing in real-world settings.
