Accordingly, recent data from the NCDR Chest Pain-MI Registry demonstrate significant NSTEMI management variability across clinical sites. This large-scale study analyzed records from 287,275 patients with Type-1 NSTEMI across 541 hospitals between 2019 and 2024. Researchers aimed to describe how the use and timing of invasive angiography vary and how these patterns associate with hospital outcomes. Consequently, the findings underscore a lack of standardized clinical pathways in real-world practice.
The study observed that 87.1% of patients underwent invasive coronary angiography. Furthermore, only 56.9% of these individuals received the procedure within the recommended 24-hour window. Among those who underwent invasive treatment, approximately 66.1% received percutaneous coronary intervention. However, a treatment paradox emerged during the analysis. Older patients and those with more comorbidities were significantly more likely to receive conservative management or experience delayed interventions. This suggests that the most vulnerable patients may often miss out on timely, guideline-directed care.
Consequently, the site-level variability for choosing an invasive strategy over a conservative one was remarkably large. The median odds ratio (MOR) for an invasive strategy reached 2.85, reflecting major differences in hospital culture and resource availability. This variability became even more pronounced during weekends and holidays. Moreover, the study established a clear link between treatment choice and survival. Using any invasive strategy was associated with significantly lower in-hospital mortality compared to conservative management, with a weighted odds ratio of 0.36.
Interestingly, this survival benefit remained consistent across all patient risk categories. Therefore, the findings suggest that reducing NSTEMI management variability through standardized protocols could improve outcomes nationwide. Standardizing the timing of angiography, particularly for high-risk and elderly populations, represents a critical area for quality improvement. In conclusion, clinicians and hospital administrators must collaborate to ensure equitable access to invasive management regardless of the day of admission or patient age.
