Dementia care systems across the globe are currently struggling to keep pace with the rising prevalence of neurodegenerative diseases. Recently, an INTERDEM policy paper analyzed the health and social care frameworks of 14 European nations. Specifically, this research identifies several systemic barriers that actively hinder timely diagnosis. Consequently, clinicians must recognize these structural challenges to improve outcomes. Furthermore, they should advocate for integrated patient pathways in their local practices. Indeed, understanding these global trends helps in refining local geriatric services for aging populations.
Notably, most European nations manage health and social care through separate administrative bodies. Specifically, Norway is the only country in the study that manages both systems together. Moreover, three out of the 14 surveyed countries still lack a formal national dementia strategy. Consequently, this fragmentation often creates a "care gap" for patients during transitions. Similarly, India faces parallel challenges in bridging the gap between medical and social support services. Therefore, creating unified policy frameworks is essential for long-term health. In addition, clinicians must identify where the current system fails the patient. As a result, families often receive fragmented care during the difficult stages of disease progression.
However, social care usage remains strictly means-tested in many regions. For example, Germany and the UK impose strict financial requirements on patients. Consequently, this creates a substantial barrier for many families and unpaid carers. Notably, Scotland represents the only nation that effectively links social care data with healthcare records. Moreover, such linked infrastructure is vital for monitoring clinical outcomes accurately. Thus, improving data connectivity remains a critical priority for health administrators. Finally, research must explore interpersonal connectivity to ensure seamless transitions for patients. Additionally, this connectivity will support unpaid carers who provide the bulk of daily assistance.

Scotland is the only country currently collecting social care data that can be effectively linked to medical healthcare data for comprehensive tracking.
