Managing transplant-ineligible newly diagnosed multiple myeloma (TIE-NDMM) has evolved rapidly over recent years. Recent clinical evidence highlights Anti-CD38 quadruplet regimens as a potent alternative to traditional triplets. Consequently, clinicians are re-evaluating frontline strategies for these patients to optimize long-term outcomes.
A comprehensive meta-analysis of six randomized controlled trials evaluated outcomes for over 2,000 patients. Researchers found that quadruplet induction significantly improved progression-free survival (PFS) compared to triplet therapy. Specifically, the hazard ratio for progression was 0.49, representing a 51% reduction in disease risk. Furthermore, these regimens achieved significantly higher rates of complete response and minimal residual disease (MRD) negativity.
This study also demonstrated a significant overall survival advantage with a hazard ratio of 0.63. However, the intensive nature of four-drug combinations leads to increased treatment-related toxicity. Notably, patients receiving Anti-CD38 quadruplet regimens experienced more frequent grade 3 or 4 infections. Serious adverse events also occurred more often in the quadruplet cohorts than in the triplet groups.
Nevertheless, the rates of severe neutropenia and thrombocytopenia remained comparable between both treatment regimens. Therefore, while quadruplets offer superior efficacy, clinicians must carefully monitor and manage toxicity in this older patient population. Additionally, subgroup analyses suggest that even frail patients derive substantial benefit from these intensive induction therapies. Overall, Anti-CD38 quadruplet regimens provide a robust new treatment paradigm for patients who cannot undergo stem cell transplantation.
Quadruplet regimens significantly improve progression-free survival (PFS) and overall survival (OS). They also lead to higher rates of complete response and MRD negativity compared to triplet combinations.
Data suggests that even frail patients benefit from these regimens. However, clinicians must vigilantly manage the higher risk of infections and serious adverse events associated with the four-drug intensity.
