This phase 4 randomized, double-blind, placebo-controlled trial assessed a single 75-mg dose of rimegepant in adults with migraine who were unsuitable for triptans because of intolerance, inadequate efficacy or contraindications. A total of 585 participants received treatment, with 570 included in the efficacy analysis. At two hours after dosing, migraine pain relief occurred in 55.9% of participants receiving rimegepant compared with 32.7% receiving placebo, an absolute difference of 23.2 percentage points (95% CI, 15.3–31.1; \(p<0.0001\)). Rimegepant was also superior across all 10 key secondary endpoints, including pain freedom and functional recovery. Adverse-event rates were similar between groups (12.5% versus 12.1%), and no severe or serious adverse events were reported in the rimegepant group. The results support rimegepant as an acute treatment option for patients who cannot use triptans.
