A systematic review and meta-analysis of randomized trials examined toric intraocular lenses for correcting corneal astigmatism during cataract surgery. Thirteen randomized trials involving more than 1,400 eyes were included. High-quality evidence showed better uncorrected distance visual acuity and greater spectacle independence with toric IOLs than with non-toric IOLs, including comparisons in which relaxing incisions were used. Residual astigmatism was also lower with toric implantation. The findings support toric IOLs as an evidence-based option when preoperative astigmatism is sufficient to affect unaided visual quality. For HCPs, the clinical value depends on accurate keratometry, axis alignment, ocular surface optimisation and control of surgically induced astigmatism. Patients should also understand that toric lenses correct regular corneal astigmatism but do not address every source of postoperative blur.
