Decompression surgery for lumbar spinal stenosis is often discussed against a background of conservative care, but randomized evidence is important when counselling patients about the likely benefit. This multicentre randomized trial compared surgical decompression with a structured nonoperative strategy. Surgery produced greater improvement in pain and function over the study period, while nonoperative treatment still provided benefit for some patients. The trial illustrates that stenosis is not simply an imaging diagnosis: symptoms such as neurogenic claudication and functional limitation determine who is most likely to value decompression. For HCPs, the key clinical question is not whether surgery is universally superior, but whether the patient's symptom burden and functional goals justify operative risk. Conservative management can include education, exercise, analgesia and activity modification, but persistent disabling symptoms despite appropriate nonoperative care make decompression a reasonable option. The paper also supports careful patient selection and expectation setting. Surgery can improve walking and pain, but it does not reverse every degenerative change on imaging. A symptom-driven approach remains the most defensible way to integrate the evidence.
