This prospective randomized pilot trial compared lacosamide with pregabalin in adults with chronic neuropathic pain. Of 72 screened patients, 36 were randomized and 29 completed the study. After 3 months, mean NRS pain scores decreased from 7.7 to 2.50 with lacosamide versus 7.6 to 4.27 with pregabalin (P<0.001). Both treatments significantly improved all SF-36 quality-of-life domains (P<0.001), with no significant between-group difference in overall quality of life. No serious adverse drug reactions occurred. Lacosamide produced greater pain reduction but was more costly because of twice-daily dosing.
