A systematic review and meta-analysis assessed the outcomes and complications of intra-arterial chemotherapy for retinoblastoma across 20 published studies involving 873 patients and 1,467 eyes. The evidence suggested that IAC can achieve substantial globe salvage, including in advanced disease, but only one included study was comparative and many studies were retrospective. Retinal detachment and transient systemic symptoms were among the reported complications, while long-term evidence on metastasis remained limited. The review therefore supports IAC as an important part of contemporary retinoblastoma care while emphasising that evidence quality is not equivalent to the strength of the published success rates. For HCPs, the clinical implication is that IAC should be used within specialised multidisciplinary ocular-oncology programmes with clear patient selection and long-term surveillance.
