A multicentre randomized trial compared femtosecond laser-assisted cataract surgery (FLACS) with conventional phacoemulsification and followed patients for 12 months. Four hundred participants were randomized, with visual acuity, refraction, corneal thickness, endothelial cell loss, adverse events and quality of life assessed. At one year, unaided distance vision was similar between the groups, and no important difference was demonstrated in major patient-reported outcomes. The study is useful because it separates the technical advantages of the femtosecond platform from ultimate visual benefit. FLACS can automate steps such as capsulotomy and lens fragmentation, but the randomized evidence does not show that every patient will achieve substantially better long-term vision simply because a laser is used. For HCPs, the choice should therefore consider surgeon preference, workflow, case complexity, cost and the potential value of a reproducible capsulotomy or reduced phaco energy. Conventional phacoemulsification remains highly effective. The trial is directly relevant to AIOC's emphasis on advanced cataract surgery and FLACS.
