Intra-arterial chemotherapy has transformed the management of advanced retinoblastoma by delivering chemotherapy directly through the ophthalmic artery. A large retrospective series evaluated 341 eyes treated with more than 1,200 infusions and examined tumour control and globe salvage across disease groups and patient characteristics. Five-year globe salvage was approximately 74% overall, although outcomes varied substantially by tumour stage. The study supports IAC as an important globe-preserving approach when delivered in experienced centres. At the same time, it is observational and therefore cannot establish comparative superiority over systemic or other focal treatment strategies. For HCPs, the key message is that globe salvage is not synonymous with useful vision. Treatment planning should consider tumour control, retinal and optic-nerve preservation, visual potential and long-term follow-up. The evidence aligns closely with AIOC's retinoblastoma globe-salvage theme within oculoplasty and paediatric ophthalmology.
