STRIVE was a large randomized placebo-controlled trial evaluating erenumab, a monoclonal antibody against the CGRP receptor, for prevention of episodic migraine. More than 950 adults were assigned to monthly erenumab 70 mg, 140 mg or placebo for six months. Both doses produced a greater reduction in monthly migraine days than placebo, increased the proportion of patients achieving at least a 50% reduction and reduced use of acute migraine-specific medication. Improvements were also seen in migraine-related physical impairment and everyday activities. Adverse-event rates were similar to placebo in the trial. The study helped establish CGRP-pathway therapy as an effective preventive option for patients with frequent episodic migraine. For HCPs, treatment choice should consider attack burden, prior preventive failures, comorbidity, patient preference and access. Erenumab is not a substitute for identifying medication overuse or addressing migraine triggers. The trial is particularly relevant to IANCON's focus on migraine and newer therapies that target disease biology rather than relying solely on traditional preventive mechanisms.
