A prospective randomized study compared endonasal endoscopic dacryocystorhinostomy with traditional external DCR in patients with primary acquired nasolacrimal obstruction. Sixty patients were randomized, and outcomes were assessed using lacrimal patency, symptoms, surgical duration and postoperative findings. Both techniques provided effective treatment, supporting endoscopic DCR as a reasonable alternative to the external approach. The choice therefore depends less on a simple difference in success and more on anatomy, surgeon expertise, the presence of intranasal disease, cosmetic considerations and the patient's preferences. Endoscopic DCR avoids an external skin incision and can address nasal pathology during the same procedure, but requires appropriate equipment and endoscopic skill. For HCPs, the study supports individualised selection rather than assuming that one route is universally superior.
