The VIVID and VISTA phase 3 trials were among the landmark studies establishing anti-VEGF therapy for center-involving diabetic macular edema. Patients were randomized to aflibercept 2 mg every four weeks, aflibercept every eight weeks after five monthly loading doses, or macular laser. At 100 weeks, both aflibercept regimens produced substantial and sustained visual-acuity gains, while the laser groups gained very little. Benefits extended beyond letter-score improvement, with more patients achieving meaningful vision gains and improvement in diabetic retinopathy severity. The study supports anti-VEGF therapy as a central component of DME management and provides evidence for a less frequent eight-week maintenance strategy after loading in appropriate patients. For HCPs, treatment decisions should still be personalised around baseline vision, OCT anatomy, systemic diabetic control, adherence and treatment burden. The evidence is directly aligned with AIOC's retina track on anti-VEGF regimens for diabetic macular edema.
