This multicenter CASPR registry analysis included 2,328 patients with embolic stroke of undetermined source. Single antiplatelet therapy (SAPT) was used in 66.0%, combination antiplatelet therapy (CAPT) in 24.1%, and anticoagulation ± antiplatelet therapy in 9.9%. In 2019–2023, CAPT and anticoagulation use increased versus 2015–2018 (26.7% vs 17.2%; 11.3% vs 5.9%; P<0.001), while SAPT declined (62.0% vs 76.9%). Neither CAPT nor anticoagulation reduced the primary composite outcome versus SAPT.
