Because acute cerebral ischaemia triggers rapid, irreversible neuronal loss—classically estimated at 1.9 million neurons per minute—every minute lost before medical contact dramatically narrows the window for intravenous thrombolysis and mechanical thrombectomy. While clinical trial protocols demonstrate extraordinary efficacy within the first 3 to 4.5 hours, community registries consistently demonstrate that most patients present well outside this critical window. A massive multicentre registry investigation from China provides critical insight into the systemic, clinical, and logistical drivers of onset-to-door time, offering actionable evidence for acute stroke care delivery in India and other developing health systems.
To evaluate contemporary patterns of emergency presentation, investigators evaluated 12,484 acute stroke admissions from the prospective China National Cerebrovascular Disease Prevention and Control Project Management Special Database. Conversely, the registry exposed severe systemic pitfalls that directly amplified prehospital delay. Inter-facility transfers emerged as the single most deleterious logistical impediment, increasing the odds of exceeding the three-hour presentation window more than threefold. Patients initially routed to primary clinics or community non-stroke facilities experienced compounding delays before secondary transfer to a comprehensive stroke centre. Furthermore, wake-up strokes and unwitnessed presentations naturally confounded symptom onset documentation, systematically delaying family recognition and subsequent emergency dispatch. Additionally, patients experiencing mild or transient neurological complaints frequently adopted an observational approach at home, deferring acute medical contact until permanent neurological deficits became firmly established.
These findings mirror the prehospital challenges encountered daily across public and private hospitals in India. Indian multicentre cohorts similarly reflect prolonged onset-to-door durations, frequently exceeding seven to nine hours on average. A substantial proportion of Indian families seek initial care from non-stroke primary healthcare centres or local general practitioners rather than comprehensive stroke-ready facilities, introducing fatal delays during inter-hospital transfers. Moreover, fragmented ambulance networks, severe urban traffic congestion, and variable literacy regarding cardinal stroke signs exacerbate these gaps. Establishing localized hub-and-spoke networks, expanding public education on the BE-FAST acronym, and prioritizing dedicated bypass protocols for emergency medical services are vital steps to ensure rapid routing directly to thrombolysis-ready hospitals.
Healthcare providers across primary, internal medicine, and emergency care must implement systematic measures to mitigate these systemic delays. First, clinicians must educate high-risk cardiovascular and diabetic patients—along with their primary caregivers—that sudden weakness, numbness, or dysphasia warrants immediate emergency transfer rather than watchful waiting. Second, peripheral medical practitioners must establish established telestroke linkages and rapid bypass algorithms, avoiding redundant diagnostic workups at facilities lacking emergency neuroimaging or thrombolytic therapy. Third, regional emergency medical networks should formalize pre-hospital notification protocols, alerting stroke resuscitation teams prior to patient arrival to collapse door-to-needle times immediately upon hospital entry.
Higher initial stroke severity, evidenced by an elevated National Institutes of Health Stroke Scale score, remains the strongest clinical predictor. Severe functional deficits or sudden loss of motor control immediately prompt bystander alarm, whereas milder sensory or minor motor symptoms frequently encourage observational delay at home.
Initial presentation to non-stroke centres causes extensive delay due to repetitive registrations, localized clinical reassessments, and secondary transportation logistics. This secondary transfer process frequently triples the likelihood of exceeding the therapeutic thrombolytic window, underscoring the necessity of direct transit to stroke-ready hospitals.
Widespread dissemination of the BE-FAST framework—evaluating Balance, Eyes, Face drooping, Arm weakness, Speech difficulty, and Time to call emergency services—empowers bystanders and families to identify stroke quickly. Rapid recognition combined with centralized emergency transport drastically reduces overall onset-to-door presentation intervals.
