Because Step 1 is now pass/fail, other metrics have gained significant weight. Program directors now look more closely at USMLE Step 2 CK scores, research productivity, and clinical evaluations. Moreover, the steady growth in the number of positions offers some hope, yet the competition remains fierce. Students must focus on holistic application building. Specifically, they should aim for high Step 2 CK scores and robust research portfolios to stand out in a field where numerical filters have evolved but high standards remain.
The transition did not significantly alter long-term match rates. While 2023 saw a temporary dip in applicants, the overall competitiveness and fill rates for Otolaryngology residency programs have remained stable from 2010 through 2025.
Applicants should prioritize achieving a high score on the USMLE Step 2 CK. Additionally, building a strong research portfolio and securing excellent clinical letters of recommendation are critical for success in the current matching environment.
